PRE-SEMINAR FORMS - These should be in before the seminar, or brought to the seminar if not sent in time. Please fill out the forms completely.

*** PLEASE NOTE THAT IF YOU HAVE PRESCRIPTION MEDICATION, THE MEDICAL FORM MUST BE FILLED OUT BY THE PRESCRIBING PHYSICIAN. IF IT IS AN OVER- THE -COUNTER MEDICATION, IT SHOULD BE FILLED OUT AND SIGNED BY A PARENT.  YOU MUST HAVE THIS FORM FILLED OUT CORRECTLY AND COMPLETELY TO ATTEND THE SEMINAR.  All medication will be collected at registration.
